Transition Program

The Transition Program is easily summed up by providing opportunities for young adults as they transition from school to Community Living.

The program is a planning service offered to young adults and their families.  Effective transitional planning begins long before the student is ready to graduate from high school.  The process is based on a life centered model developed through input of the student, family and other significant people in the student's life.  Planning for the future is crucial for students with developmental disabilities as many families are unaware that services are not automatically provided once they leave the school system. The result is that support systems are reduced to only the parents or family. Other support services are unavailable until referrals are made. Waiting lists for support and the funding that go with it are lengthy.
